[Bug 76944] Improve yelp index layout

Public bug reported:

Binary package hint: yelp

I'm going to attach a debdiff which improves the yelp index layout, in
line with changes made to the ubuntu-docs document structure for this
release. It produces something like this: http://mdke.org/tmp/yelp.png

It has at least the following problems, some of which I am not able to
fix (it's my first time with a vaguely serious patch):

* The new text added in stylesheets/toc2html.xsl needs to be localisable - I don't know how to do this.
* The debdiff itself has some strange problems: some files appear only with a timestamp change, and there is some strange stuff at the end. I suspect these were caused by the fact that I had a number of attempts to make my changes, but I don't know how to fix it.
* The bullet points are laid out badly.
* It doesn't include a dependency which we will need to add: if these changes are made we will need yelp to depend on ubuntu-docs, because the links in the "Common questions" section will point to documents shipped with ubuntu-docs.

Anyway, any feedback is very welcome.
