[Bug 40271] Unable to create new addressbook

Description:
1: open evolution
2: click contacts
3: right click on Personal addressbook and select new address book
4: Type in name for new address book 

The "OK" button is greyed out. There is no way to create a new
addressbook.

New contacts, calanders, and tasks work.

This is on a clean flight 5 install with dist-upgrade before logging
into a fresh account.

evolution-webcal = 2.5.90-0ubuntu1
evolution-data-server = 1.6.1-0ubuntu1
