This works fine for me with xpdf rather than totem.  Here is what I do:

1. Visit www.tldp.org/guides.html
2. Use the right-button context menu, clicking on `Introduction to Linux - A
Hands on Guide', item 4, PDF.
3. From that menu, select `Open Link in New Window'.
4. A blank Firefox window appears, followed shortly afterwards by a dialogue box
`Opening intro-linux.pdf'.  The dialogue box has focus.
5. Left-click on the X at the top right of the blank firefox window.  It
disappears, leaving the Opening dialogue with focus.
6. The dialogue offers `(*) Open with [ /usr/bin/xpdf (default) ]', which I
accept by clicking on OK.
7. The dialogue disappears; a download window briefly appears and is soon
covered up by xpdf showing the document.
8. I close xpdf with the X at the top right.  This shows the `Downloads' window,
which must be closed separately.

Perhaps this is a problem with the totem plugin.
