Thanks for your help finding that one.  It sounds very much like the problem.

If this is the case, GNOME:
1. once (in Warty and maybe Hoary) created files for user customizations as
~/.local/share/applications/<appname>.desktop
2. always (in Warty, Hoary, and Breezy) includes a statement to suppress that
version of the icon appearing
3. currently (since Breezy) causes a user's customizations to override the
system-wide menus

This looks like it is going to be a problem for some users migrating from Warty
and Hoary.

I also note that this bug is still open upstream, so I'm wondering how Rhythmbox
is affected by Totem doesn't seem to be.
