Some positive feedback on the responsiveness of bzr(explorer)

Two other common cases:

- Executables/libraries being demand-paged in, rather than completely
  read on load.  Actually, technically speaking, this is really just a
  special case of:

- Files accessed by mmap; you take a page fault when you access a page
  that isn't filled in.
