bzr 2.2.1: Thursday 16th September

It's just over a month since the code freeze for 2.2.0 and I propose
we do a 2.2.1 next Thursday.

The main changes in this would be:

 * an updated and more complete "what's new in 2.2?" document
 * a few fixes for bzr add <http://pad.lv/192859>
 * updates to urls in the documentation

The Windows installers will probably also include a qbzr 0.19.1 which
bialix says he will release over the weekend.
