So I've been thinking about this some more, and I wanted to point out
that *today* the disk cache is not optional. Specifically, if
http_sql_dir has not been configured SQL_DIR is set to a temp directory
(which isn't cleaned up when the process exits...)

So making the disk cache mandatory isn't a big overhead over the
existing code...
