[MERGE] Add format registry for send formats.

This patch adds a format registry for "bzr send" formats. This should
allow bzr-git and bzr-hg to register custom formats for their
respective patch formats. 

Like my bzr serve --protocol patch, it puts a registry inside a cmd_
implementation (cmd_send.format_registry) but I think this is the
cleanest solution.
