[MERGE] Add "--fixes lp:" to the launchpad plugin documentation.

Hi,

Marius didn't know about "bzr help bugs", so he went to "bzr help
launchpad" for how to link a launchpad bug, but it doesn't specify
that it can do that. The attached merge directive fixes that.

While I had my editor open I made a couple of tweaks. I believe
that register-branch will be used far less frequently than the
lp: directory service, so I put it last in the list.

I also removed the "///" in an lp: example, as I find it redundant.
A Launchpad person may tell me why it is important, but I've never
needed it.
