New patch attached addressing many of these issues. As suggested
in another email, I've gone with 3 topics: formats, current-formats
and other-formats. The first one is an overall introduction that
explains how to select a format, including the issues around
plain vs rich-root. The latter two topics are plain lists giving
the formats in the order they were introduced. (This allows the
descriptions to build on one another.)
I've also improved the descriptions for most of the current formats.
Hopefully they are better - and clearer - now.
