[MERGE] launchpad-login sets default username for ssh-based Launchpad connections

Hi all,

This patch implements default username support on sftp and bzr+ssh for
the launchpad-login plugin.

New launchpad-logins will set values in both bazaar.conf and
authentication.conf.  Existing users will have their authentication.conf
automatically upgraded the next time their login is used (e.g. when
doing an lp: lookup).

For now, both values will be required, and must be in sync.  In the
future, we may remove the bazaar.conf setting.  And we probably *will*
remove usernames from the urls produced by lp: lookups.
