[MERGE] updated faster-iter-changes

This is an updated patch with Johns comments so far actioned.

One exception is moving the oython versions to _dirstate_helpers_py.py;
this breaks horrendously, I spent about 30 minutes fiddling, but short
of not doing function=top_level_function, which caches for better
performance, its not at all easily doable.

And as it looks like win32 might not get an accelerated build straight
off, I'm not quite ready to make the  python version slower just yet.
