[MERGE] Btree Indexes

Lets rename the helpers to _btree_serialiser_CODE, as they do both
parsing and serialisation now.

I think we should not include 
FixedMemoryGraphIndex 
:) - its really only something I needed for converting some GB sized
indices - normally anyone that has successfully creating a GraphIndex
can convert them easily.

The first line in chunk_writer.py (and perhaps others) needs updating as
its not in a plugin anymore.

Other than that, I'm happy.
