[MERGE][1.6] Cause a full index read when a single index query is larger than 1/20th the index capacity.

This improves log back to 1.3 speeds, largely fixes 'log file', and also
appears to benefit annotate.

Please give it a whirl if 1.6 has been feeling sluggish to you.
