[MERGE] docutils still has trouble with a dot in an option name

There is a hack in tools/rst2html that allows docutils to work when a 'dot'
appears in an option name.  The hack in rst2html explicitly checks the
docutils version is <= 0.4.1 before applying the patch - but the docutils
trunk shows this hack is needed for all version of docutils (ie, docutils
has not changed in this regard at all)

Does anyone recall why the expectation is that docutils 0.4.2 fixed this?
Either way, the attached patch, which unconditionally applies the hack,
makes things work for me.
