[MERGE] Move ignores updating functionality out of cmd_ignore

The attached patch moves the code that adds ignores to the ignore file out of 
the cmd_ignore class into a separate function in bzrlib.ignore. This
should make it easier for other users of bzrlib to update the ignores
file.
