RFC: GraphIndex2

So, I'm assembling some things to address in the index layer.

My notes so far (early days):

Please jump in with things we should keep or to watch out for etc.

I'd like to assemble some things to measure in the current code base
which can be used to assess problems/designs. 

E.g. the repeated-key-lookup-rate during various operations.
