[merge] various developer process docs

* merge some outdated but useful docs from the wiki
* update index pages
* update release process docs

I see that at the moment we have the developer guide in
doc/developers/HACKING.txt, but it's built into
doc/en/developer-guide, which is a bit schizophrenic.  (In particular
it makes it hard to get hyperlinks right because the html is in a
different place to the input source.)  Maybe we should just move all
of this under doc/en/?
