[merge] remove code deprecated prior to 1.1

This kills some quite old code that's been deprecated for a long time.
(Over a thousand lines in fact.)

A couple of things were only deprecated in docstrings or comments but I
think they're fairly safe.

I've tried to be careful about checking which tests were specifically of
the deprecated interfaces, and which ones ought to be updated.  A double
check of that would be useful.

I have to say I really like the way you can still test deprecated
interfaces but have to be explicit about doing so, and therefore
consider whether you need a new test too.
