[bzr-loom:MERGE] Allow combine-thread to delete the bottom thread (#194274)

Hi Robert,

This patch fixes bug 194274, allowing the bottom thread to be deleted.

I also moved responsibility for finding thread indices onto the
LoomState object.  I think it would be nice to hide the threads behind
the LoomState, in general.

And I updated the copyright dates as accurately as I could.

Still not happy with the "combine-thread" name.  How about "delete-thread"?
