[MERGE] Add thread_name_suffix to SmartTCPServer (protocol v3 patch 1/7)

This is the first in a series of patches that implement version 3 of the smart

Allow setting a thread_name_suffix, which is useful for identifying which test
caused a traceback in a thread.
