This was a small regression introduced with some changes to log. Basically, the
api assumed that there would be no ghosts. It now properly skips ghost nodes
when determining the revisions that modified a file.

The bug is marked as Critical for 1.4 (as it breaks all 'bzr log XXX' for
bzr.dev). The change is small enough that I don't think it will be a problem.
