[MERGE][1.0] Clean-up of User Guide for 1.0

This patch improves the correctness and the quality of the User Guide
ready for 1.0. Changes are:

* switch works for normal checkouts now, not just lightweight ones
* 'Specifying revisions' is referenced from Core Concepts,
  updated to include tag: and submit: and refers to the User Reference
* Several broken links fixed
* Content more friendly to users on Windows
* Minor clean-ups to the smart server doc
* Expansion of "Exporting version information" to be less
  Python-centric
* "Choosing a shared repository layout" referenced from the main
  text and moved into the appendices.
