[MERGE] Generic bug tracker configuration

I think simple action such as configuring a bug tracker should not
require writing a plugin or patching bzrlib. This patch adds support for
configuring bug trackers with custom URL schemes without writing any
code. See the changes in doc/en/user-guide/bug_trackers.txt for some
examples.
