[MERGE] stop checking content and revision uniqueness during knit insertions in commit

This is an updated patch as per Ian's feedback.
The content checks are now able to be disabled rather than just removed.
This is not quite as safe as having them always there, as there is
enough layers that someone could write a bad tree that will do the wrong
thing, however for our implementations it's entirely safe - the caveat
only applies to code outside the regular commit paths.
