OK to merge doc changes into 0.90?

James,

As Release Manager for 0.90, you get the final say as to whether the
"doc into directories" changes get merged into 0.90 or otherwise. I
believe they are low risk, being mostly doc changes.

To summarise the status:

1. I released my "final" patch last Tuesday which addressed feedback
   from poolie on what was required before it could be merged.
2. Alex voted tweak with changes needed for the Windows installer.
   He subsequently submitted an updated patch with a better version
   of that fix including my stuff. (As I was on leave.)
3. I've now checked his change and approved it.
