[MERGE] Send actually *sends* the merge directive

Hi all,

This patch updates the send command to actually send the merge directive,
using either using the editor, or using a mail client specified by the user.

Current supported clients are "editor" (the default) and "thunderbird" (Mozilla
Thunderbird / Icedove).  This allows everyone to use the functionality, and
also makes me happy.  If you want your favourite client supported, please "bzr
send" a patch!

It introduces the following config variables:
mail_client (the mail client to use)
submit_to (default address to submit changes to)

It also adds documentation for submit_branch and public_branch.  (Doh!)

Yes folks, the dream of "bzr send"[ENTER] is at hand.  Now will you stop using
"bzr bundle"?  Please?

And can you guess which mail client I'm sending this with?  Why "editor", of
course.
