[MERGE] change extension of generated html documentation from htm to html

couldn't see an entry for this change itself.

The html link is correct here but the file has been renamed HACKING.txt,
not just HACKING. Please fix the text here and not just the link.

Any reason these 2 lines are just removed and not replaced with matching
html ones?

I like the formatting improvements you made to the doc indexes but you
missed the man page. Assuming my User Reference change gets though PQM,
it will be slightly different anyway when you merge.

On the whole, it looks fine to me bar the notes above. Thanks for this.
