[MERGE] key-length GraphIndex adapter

This adds 'GraphIndexPrefixAdapter' to the bzrlib.index api. This
adapter allows a 2-key-element index to be accessed as a single-key
element index, for mapping multiple KnitGraphIndices onto a single
GraphIndex.
