[RFC] make all-in-one formats upgrade-only

Hi all,

I just managed to waste a good few hours trying to sort out why bundle
format 4 didn't work with weave repositories.

The good news is that format 4 *does* work with weave repositories.  So
long as you use "metaweave", not "weave".  Our all-in-one formats don't
support that the update command, but the bundle test suite relies on it.

The metadir formats were introduced with Bazaar 0.8, a bit more than a
year ago.  It seems like the all-in-one formats don't have good test
coverage, and without that, I think it makes sense to relegate them to
upgrade-only status.
