[MERGE] documentation from the london sprint

This is documentation from the london sprint on add, annotate, gc and
revert as well as some extra push-pull notes.

I'm aware its not very polished, so this may be more of a discussion
prompter than a ready-to-merge, but - if it gets +1, I'll happily merge
it and incrementally improve it.
