[MERGE] bzr help disambiguation and bzr help PLUGINNAME

This adds support for bzr help to identify when topics from areas like
'registered topics' and 'plugins' and 'commands' overlap, and report
this to the user.

Its not 100% factored, but I think its only a little ugly and not worth
refactoring right now, as it would involve somewhat more imports during
'bzr rocks' - and I wanted to avoid that.

It fixes bugs 107656 and 50408.
