HPSS: the rest

This subset of the full hpss diff contains everything not in the other subsets
(almost; I have elided a couple of irrelevant changes that have also removed
from the hpss branch).

Most of these are changes are pretty minor, and so I hope uncontroversial.

An exception: the bzrlib/smart/medium.py changes look obviously bogus to me, so
I'll fix that shortly.

This is the last subset of the full hpss changes.  Enjoy!
