[MERGE] High-performance Smart Server

This diff is the remaining unreviewed parts of the hpss branch at
http://bazaar.launchpad.net/~bzr/bzr/hpss.

I'll follow up with this diff broken up into parts that can be sensibly reviewed
independently (even though it needs to all merge at once for tests to pass).

There are probably a few small pieces that could be merged and reviewed
independently, but I think we're at the point of diminishing returns.  So, here
it is!

Any review feedback at all is helpful.
