[MERGE] Fix encoding of Already Versioned error

As it turns out, when you manually specify the message for BzrError, it
may only be in utf-8, not unicode.  This patch fixes the problem in
Inventory.add

Also, NEWS had two BUGFIXES headings, so I zapped one with my, err,
bugfixes-zapper.
