Bundle Buggy now supports patch filenames

Hi all,

Just wanted to let you know that BB now stores and uses the original
patch filenames, so that when you download them, your browser can
suggest a unique, and hopefully-descriptive filename, rather than
'patch.txt'.

This only applies to new patches, because I had to add a new column to
the database table.
