[MERGE] format-registry deprecations

Hi all,

This patch
- - allows make_branch_and_tree's format parameter to be a string, which
will be looked up from the FormatRegistry
- - updates the upgrade tests so they don't use
Repository.set_default_format / BzrDir.set_default_format (and use more
modern APIs in general)
- - deprecates Repository.set_default_format
- - deprecates BzrDir.set_default_format
- - introduces FormatRegistry.set_default_repository as an alternative to
Repository.set_default_format
