I think it makes sense to have requests for:

.bzr/branch/foo

go through

.bzr/smart

Rather than .bzr/branch/.bzr/smart

I'm not sure why you disagree with that. I can say that requests for:

repo/.bzr/repository/foo

should be different from

repo/branch/.bzr/branch/foo

But I think it is reasonable to have a logical '.bzr/smart' "file" next
to '.bzr/branch-format', and not to have a simulated '.bzr/branch/.bzr'
directory.

It seems that there should be one 'smart' file for every real '.bzr'
directory.
