[rfc] remove foo.bar; add foo.bar -> make this no-op?

Still, that could be cool to have a warning when "bzr add"-ing a file
which has just been "bzr rm"-ed, pointing to revert.

A user not aware of the notion of file-id will think that rm+add is a
no-op, it's good to let him know it's not.
