[MERGE] Remove things deprecated in 0.8 and 0.7

Hi all,
This bundle removes a bunch of deprecated functionality, including
deprecated parameters.
Removing the deprecated parameters changes argument order, but since the
parameter was deprecated, everything following the parameter should have
been passed in as a keyword argument anyhow.
I did find a couple places in the code where this was not the case, but
nothing in any of my usual plugins.
The one thing I didn't remove was WorkingTree.iter_conflicts. This is
because iter_conflicts does exactly what remerge needs to do, and it
doesn't seem sensible to remove useful functionality, because we'd just
have to reinvent it elsewhere. Should we just undeprecate it, or should
we rename it, too?
