[MERGE] no default ignores

This small patch removes the default ignores list.
It exposes the old list as 'bzr ignore --old-default-rules' so people
can see what we accumulated.
Its somewhat unsatisfactory as there is no way to get an old tree (from
before this is done) to lint clean - but if we carry forward with Jan's
larger ignore patch, we will end up fixing that eventually.
Martin and I agree that this while not fully satisfactory is a
reasonable compromise given considerations of getting something done and
taking some pain earlier rather than later in terms of the number of
users affected.
