.bzrignore and bzr ignore are confusing

I agree that this is 1) clear, and 2) unlikely to be construed as
surprising behaviour.

> bzr ignore Makefile   => ignore Makefile in general

This, however, is likely to break (2); I think it will give results
that are surprising to some users.

As ignoring a basename in the entire tree doesn't have an equivalent
in "bzr add", I think it would be good to require an explicit
command-line switch for it.
