A number of things need to be fixed for this app to be ready for inclusion:
 - debian/changelog should have a real name and real email address, also should it target 'precise' instead of 'unstable'
 - debian/control should also have real name and real email address
 - debian/control should not build-depends or depends on "some-other-package"
 - debian/control should point to a real homepage
 - it will be good to have long description in debian/control as well
 - consider depending on all the tools your app needs
 - it seems like all your source code files are licensed under "public domain", so you want to update debian/copyright accordingly
 - all the files of your app are installed into /usr when they should be installed into /opt - http://bazaar.launchpad.net/~ubuntu-app-review-contributors/ubuntu-app-reviews/download-monitor/view/head:/debian/rules might serve as an example for how debian/rules could look
